Allied health support for older Australians living at home

Support at Home is the Australian Government’s in-home aged care program designed to help older people remain independent, safe and well at home for longer.

At Bloom Healthcare, we provide personalised allied health support for older Australians, families, carers and care providers. Our team can help with practical, goal-based therapy and assessments that support mobility, daily living, independence, communication, nutrition, emotional wellbeing and overall quality of life.

Our aged care allied health services can be delivered at home, in the community or through telehealth where appropriate.

What is Support at Home?

Support at Home is a government-funded aged care program that helps eligible older Australians access services, equipment and support to continue living at home.

It replaced the Home Care Packages Program and Short-Term Restorative Care Programme from 1 November 2025. The Commonwealth Home Support Programme will continue separately for now and is expected to transition later.

Support at Home is designed for people who need coordinated support at home. This may include help with everyday activities, allied health, nursing, personal care, home modifications, assistive technology or short-term support after a change in health or function.

For many older Australians, the goal is simple.

To stay living at home safely, confidently and with the right support around them.

 

Who is Support at Home for?

Support at Home may be suitable for older people who need help to remain living independently at home.

This may include people who:

    • are finding everyday tasks harder
    • have reduced mobility, strength or balance
    • have experienced a fall or recent hospital admission
    • are managing chronic health conditions
    • need support after illness, injury or surgery
    • require equipment or home modifications
    • need help with communication, swallowing or nutrition
    • are experiencing changes in memory, mood or confidence
    • need coordinated aged care support at home

Eligibility is determined through an aged care assessment. This assessment looks at the person’s needs, goals, current situation and what support may help them live safely and well at home.

How does Support at Home work?

Support at Home begins with an aged care assessment through My Aged Care.

If a person is approved for Support at Home, they receive a support plan that outlines the services and funding they have been approved for. This may include ongoing support, short-term pathways, assistive technology, home modifications or other approved services.

The person can then choose providers to deliver the services that match their assessed needs and support plan.

Bloom Healthcare can work with older Australians, families, carers, referrers and care providers to deliver allied health services that support independence, safety and wellbeing at home.

What services are available under Support at Home?

Support at Home includes a defined list of services designed to help older Australians remain living at home for longer.

These services generally fall into three main categories.

Clinical Supports

Clinical Supports help maintain health, function, safety and wellbeing.

These may include allied health and nursing services that support a person’s ability to live safely and independently at home.

Clinical Supports may help with mobility, falls prevention, daily living skills, home safety, strength, swallowing, communication, nutrition, mental health and management of chronic conditions.

Independence Services

Independence Services help older people manage everyday activities and stay connected with their community.

These may include support such as personal care, transport, respite and assistance with daily routines.

For some people, allied health may also support independence by helping them build confidence, improve function, reduce risk and continue doing the activities that matter to them.

Everyday Living Services

Everyday Living Services help maintain a safe and manageable home environment.

These may include domestic assistance, cleaning, gardening, shopping, meals, home maintenance and repairs.

While Bloom Healthcare does not provide every everyday living service, our allied health team can help identify risks, recommend practical strategies and support care planning where a person’s home environment affects their safety, function or independence.

Allied health services under Support at Home

Allied health can play an important role in helping older Australians stay independent at home.

At Bloom Healthcare, our aged care allied health services are person-centred, practical and focused on meaningful outcomes. We work with each person’s needs, goals, home environment and care team to provide support that is tailored to their situation.

Occupational Therapy helps older adults manage daily activities with greater safety, confidence and independence.

Physiotherapy helps older adults improve movement, strength, balance, mobility and physical function.

Exercise Physiology supports older adults through tailored exercise programs that are safe, evidence-based and matched to their health needs.

Speech Pathology helps older adults with communication and swallowing difficulties.

Psychology and Counselling can support the emotional and psychological challenges that may come with ageing, illness, loss, life transitions or reduced independence.

Assistive technology and home modifications

Support at Home includes an Assistive Technology and Home Modifications pathway for approved participants.

Assistive technology may include equipment or products that help a person complete daily activities more safely or independently. This may include mobility aids, bathroom equipment or other items that support function at home.

Home modifications may include changes to the home environment to make it safer and more accessible. This may include grab rails, ramps, bathroom changes or other modifications based on assessed needs.

Bloom Healthcare’s Occupational Therapists and Physiotherapists can support assessments and recommendations for equipment, safety and function at home where this aligns with the person’s needs and support plan.
