In plain English

Aged care allied health supports independence and quality of life.

Bloom’s aged care clinicians help older people maintain or regain the abilities that make everyday life possible—moving safely, managing daily activities, communicating, eating and drinking, staying active and looking after emotional wellbeing. Services are personalised and can be coordinated with family, carers and the person’s registered aged care provider.

One enquiry can help an older person, family or provider identify the right discipline, funding arrangement and next step.
What Aged Care actually is

Clinical expertise connected to the life you want to live.

We look at your strengths, health, environment, routines and priorities. From there, we agree on practical support that is relevant, understandable and able to be reviewed.

1

Stay safely at home for longer

Assessment considers how to stay safe at home for longer by assessing the home environment, routines and the support already in place.

2

Keep doing everyday activities

Recommendations for everyday activities are designed to be practical for the older person, family, carers and care team.

3

Reduce falls risk and build confidence

Support builds toward reducing falls risk and building confidence through relevant skills, equipment, environmental changes or education where appropriate.

4

Make communication and mealtimes safer

Progress toward making communication and mealtimes safer is reviewed against the older person’s priorities, safety, comfort and quality of life.

Common questions

Answers before you begin.

If you are unsure whether aged care is the right service, tell us what is happening. You do not need to diagnose the solution yourself.

Ask us a question

Do I need to know which allied health service I need?

No. Tell our team what has changed or what you would like to make easier. We can help identify the most appropriate discipline and next step.

Where can appointments take place?

Bloom primarily provides support at home and in the community. Telehealth may be available when it is clinically suitable. Clinic appointments may be available case by case.

Can a family member or carer be involved?

Yes. With the older person’s consent, family members, carers and other trusted supporters can join conversations and help put strategies into practice.

Can Bloom work with my existing aged care provider?

Yes. Bloom can work with older people, families and registered Support at Home providers. The funding and service arrangement must be confirmed before services begin.

What if I am not receiving government-funded aged care?

Contact Bloom to discuss available pathways. Depending on your circumstances, services may also be accessed privately or through another relevant funding pathway.

How quickly can support begin?

Availability varies by discipline and location. Sharing your postcode, preferred service and urgency helps our team give you a clearer answer.
